Get a Pet Insurance Claim Form

You can download an ASPCA Pet Health Insurance claim form by clicking here.
The form is a PDF file, so you'll need Adobe Acrobat Reader to open it.

Please send the completed form with your receipts to the address, fax number, or email on the form within 180 days of the date of service.

Helpful Claim Tips

• Please use only one claim form per pet, per accident or illness.
• You can fax, mail, or email your claim and itemized invoice to us – whatever works best for you.
  If you use email, you can scan and attach your receipts.
• Help make sure your claim is processed as quickly as possible by completely filling out the form.
• Don’t forget to sign your claim form at the bottom. Your veterinarian’s signature isn’t required.
• Make sure you include your plan number on any documents you send us.
